1. General Provisions

This regulation establishes the general conditions for using medical services provided by Gabinety Borki. By using our services, you accept the following conditions.

2. Services Offered

Our medical center offers the following services:

• Dental and periodontal services
• Cardiology consultations and diagnostics
• Ophthalmology services
• Traumatology and orthopedics
• Other medical specialties as available

3. Appointment Rules

Appointments are managed as follows:

• Appointments can be requested by phone or through the web form
• Appointment confirmation will be sent by SMS or email
• Cancellations must be made at least 24 hours in advance

4. Patient Rights

As a patient, you have the right to:

• Complete information about your health status and treatments
• Access to your medical records
• Confidentiality of your medical data
• File complaints

5. Patient Obligations

The patient agrees to:

• Provide truthful information about their health status
• Arrive punctually to scheduled appointments
• Respect medical staff and facilities
• Pay for services according to established rates

6. Payments

Payments are made by cash, debit/credit card, or bank transfer. We offer financing up to 60 months interest-free.

7. Liability

The medical center is not responsible for consequences arising from patient non-compliance with medical recommendations.

8. Complaints

Complaints can be submitted in writing or by email. They will be addressed within a maximum of 30 days.

9. Final Provisions

This regulation is subject to Polish and European Union law. Any modifications will be communicated to patients.
